Patient's Rights and Responsibilities


Patient Rights…

  • Patients have the right to be fully informed of his/her rights and responsibilities.
  • Patients have the right to be treated with respect, courtesy, consideration and dignity.
  • Patients have the right to exercise their rights without being subjected to discrimination or reprisal.
  • Patients have the right to voice grievances regarding treatment or care that is (or fails to be) furnished at this Center.
  • The patient has the right to request changes in service or staff.
  • Patients have the right to be fully informed about a treatment or procedure and the expected outcome before it is performed.
  • Patients have the right to receive care in a safe setting
  • Patients have the right to be free from all forms of abuse or harassment.
  • Patients have the right to appropriate personal privacy and privacy concerning their Health Information as described in The Health Insurance Portability and Accountability Act (HIPAA) of 1996.
  • Patient disclosures and records are treated confidentially, and, except when required by law, patients have the right to approve or refuse their release.
  • Patients are provided, to the degree known, complete information concerning their diagnosis, evaluation, treatment and prognosis. When it is medically inadvisable to give such information to a patient, the information is provided to a person designated by the patient or to a legally authorized person.
  • Patients have the right to participated in decisions involving their health care, except when such participation is contraindicated for medical reasons.
  • Patients have the right to request and receive information regarding the services available at this Center and the credentials of the Physicians and Staff providing such services.
  • The patient has the right to receive information necessary to give informed consent prior to the start of any procedure or treatment.
  • The patient has the right to refuse treatment within the confines of the law and to be informed of the consequences of his/her actions.
  • Patients have the right to request a different physician.
  • Patients have the right to information regarding advanced directives and to make informed decisions regarding the patient’s care.
  • The patient has the right to care that reflects consideration of personal values and belief systems, and is allowed to express those spiritual beliefs and cultural practices that do not harm others or interfere with the planned course of medical treatment.
  • The patient has the right to disclosure of his/her physician’s financial interest (if any) in this Center.

 

 

Patient Responsibilities…

  • The patient has the responsibility to observe prescribed rules of the Center for their stay and treatment and, if instructions are not followed, forfeits the right to care at the Center and is responsible for the outcome.
  • The patient has the responsibility of notifying the Center about any living will, medical power of attorney, or other directive that could affect his/her care.
  • The patient has the obligation to promptly fulfill the financial obligations to the Center.
  • The patient has the responsibility for being considerate of other patients and personnel and for assisting in the control of noise, smoking and other distractions.
  • The patient and family are responsible for the respect of property of others and the Center.
  • The patient has the responsibility to report whether he / she clearly understands the course of treatment and what is expected of them.
  • The patient has the responsibility of following the treatment plan prescribed by his/her provider and for his /her actions should he / she refuse treatment or does not follow the physician’s or Center’s instructions.
  • The patient has the responsibility for keeping appointments and, when unable to do so for any reason, of notifying the physician and Center.
  • The patient has the responsibility of providing a responsible adult to transport him/her home from the Center and remain with him/her for twenty-four hours, if required by his/her provider.
  • The patient and family have the responsibility to inform the physician and Center personnel of changes in his / her medical condition which may affect the outcome of the procedure to be performed.
  • The patient has the responsibility to provide complete and accurate information to the best of his/her ability about his/her health, any medications, including over-the-counter products and dietary supplements and allergies or sensitivities.
